Rural Health Clinic
The Rural Health Clinics program was established to stabilize access to outpatient primary care in underserved rural areas and encourage the use of physicians, physician assistants, nurse practitioners, and certified nurse midwives. Under the program the U.S. Center for Medicare and Medicaid Services (CMS) designates private and non-profit clinics meeting certain conditions for certification as Rural Health Clinics (RHCs). RHCs are eligible for enhanced Medicare and Medicaid reimbursement. To learn more about the Rural Health Clinic in Plainview, please visit their website.

Diabetes Education
Our mission is to provide quality comprehensive diabetes self-management training and education for the people of the Southwest. We believe that patient and family education is essential and the right of every person. The responsibility belongs not only to the multidisciplinary health care team, but also to the person and family affected by this disease. Our goal is to reduce the risks of developing the long-term complications associated with diabetes.
Left unmanaged, diabetes can lead to several medical complications including: blindness, kidney disease, heart disease, stroke, nervous disease, amputation, and impotence. The good news is that you reduce the risk of developing complications by taking control of the disease. The Outpatient Program at Covenant Hospital Plainview consists of a series of comprehensive educational classes that teach both the patient and their family self-management skills to reduce the risk of complications.
Classes are offered monthly with follow-up provided following the completion of the series of classes. (A free support group also meets monthly.) Together, you and your diabetes team will review your progress in managing the disease and enjoying an active healthy life.
