Covenant Center for Outpatient Diabetes Education
The Covenant Center for Outpatient Diabetes Education Center is open Monday thru Friday, 9:00 a.m. until 6:00 p.m. Our Services are available to anyone who is interested in learning more about Diabetes. The Diabetes Center is located on the 4th floor at Covenant Hospital Plainview. Our team consists of your doctor, a Nurse Educator, a Dietician, and YOU. "Let's Do Diabetes Care Together"
"Let's Do Diabetes Care Together" is a comprehensive program recognized by the American Diabetes Association. This program is targeted for newly diagnosed patients with diabetes, or for patients whose blood glucose or A1C is out of control. The comprehensive program consists of an initial appointment with a nurse educator and a dietician and then four group classes for a total of 10 hours of training. Follow-up appointments and an exercise program are also included in our services. We have classes to meet almost anyone's schedule. Our morning classes meet from 9:00-11:00 a.m. and our evening classes meet from 6:00-8:00 p.m., alternating every other month. Medicare and most major insurance companies usually cover much of the cost of Diabetes Education with a referral from your physician. Click here to see the 2007 class schedule.
"Diabetes Support Group"
The "Diabetes Support Group" is a community service provided by our Diabetes Center. It meets the 3rd Monday night of every month at 7:00 p.m. in the Max Gabriel Conference Room. There is no charge for this program. A wide variety of speakers and topics are presented at these support groups to keep everyone updated on the latest in diabetes care. The class is a good way to meet new people, learn new information and have a little fun. No reservations required. Click here to see the 2007 support group schedule.
